US ECRI Releases: Top Ten Medical Technology Hazards in 2017
On November 7, 2016, the Emergency Care Research Institute (ECRI) released the “Top Ten Medical Technology Hazards 2017†on its official website to remind medical institutions of the importance of medical device and medical equipment safety.
To reduce the potentially fatal risk of intravenous infusions, most infusion pumps incorporate safety mechanisms, and while these mechanisms greatly improve the safety of infusions, they do not eliminate all errors.
Using contaminated medical devices can result in patient infection or instrument malfunction. Taking duodenoscopy as an example, major medical accidents in recent years have been related to it. However, duodenoscopes are not the only ones to be aware of, but also include complex, reusable instruments such as endoscopes, hollow drills, arthroscopic razors, and more.
Complex reusable medical devices can be difficult to clean, disinfect, or sterilize (ie, post-treat), and sometimes even lingering contamination makes it difficult to accurately detect the instrument. In addition, inadvertent cleaning steps are also a cause of contamination of the device.
For a ventilator that provides life-sustaining treatment, missing an alarm can be fatal. There are two main potential problems: 1) the employee is experiencing alarm fatigue, which becomes overwhelmed, distracted or numb; 2) the alarm notification fails and the alarm is not effectively communicated to the employee.
