Current Hp detection may have false positives

Current Hp tests may have false positives. Italian Brandi et al reported that non-Helicobacter pylori (Hp) urease-positive bacteria were found in gastric juice and gastric mucosa of low-acid patients. The results indicate that clinically, low-acid patients with a gastric mucosal rapid urease test (RUT) and a urea breath test (C-UBT) for the diagnosis of Hp infection may have false positives. Hp is the only bacteria known to carry active urease in the human stomach. Therefore, gastric mucosal RUT and C-UBT are used to rapidly detect Hp infection. However, it has been reported that there is a false positive result of C-UBT in patients after gastrectomy, and it is speculated that there is a urease-positive bacteria other than Hp in the digestive tract.
The study included 25 patients with low acidity, 15 of whom were treated with omeprazole for digestive diseases, 10 were autoimmune atrophic gastritis, and 10 patients with normal gastric acid (control group) were collected for gastric juice and gastric mucosa. Hp and other bacterial cultures.
All patients underwent gastroscopy at 8 am and retained biopsy specimens of relevant gastric fluid (all patients) and gastric mucosa (12 in total, 6 from the corpus, and 6 from the antrum). All specimens were subjected to standard requirements, anaerobic culture and Hp specific strain screening.
In addition, urease activity analysis was performed on all strains. The urease-positive strains were placed in an acidic environment (pH 7, 7, 3, 2, respectively) with or without urea for 0, 20, 30, and 60 minutes, and their viability was tested.
The results showed that no other bacteria were found in the stomach of the control group except Hp. Ten urease-positive non-Hp bacteria were isolated from 6 patients with low acidity (including 1 patient with autoimmune atrophic gastritis, 4 patients with erosive reflux disease, and 1 patient with Zhuoai syndrome). However, Staphylococcus capitis urealiticum is similar to Hp (all can survive in any acidic environment with or without urea, urease activity is the strongest), most non-Hp urease-positive bacteria urease activity is not as good as Hp. —— Information from: Meditech Medical Network
